Athletic Night Information - In's & Out's of Patriot Athletics

2025-2026 SCHOLAR/PARENT ATHLETIC PACKET

Required Forms & Documents included in the 2025-2026 Athletic Packet:

• 1) EL2 Physical Evaluation Form
• - Must be completed and signed by a licensed Doctor (MD, DO, or Nurse Practitioner)
• - Only the official EL2 form is accepted — school health forms will not be accepted
• - Must be dated within the past 12 months
• 2) EL3 Consent and Release Form
• – Fully completed and signed by both the parent/guardian and the scholar
• 3) Scholar/Parent Athletic Contract
• – Signed by both the scholar and the parent/guardian
• – Acknowledges expectations, responsibilities, and commitment
• 4) Extracurricular Behavior Contract
• – Signed agreement outlining behavioral expectations for participation in athletics

Eligibility & Account Requirements:

1. No Service Hour Holds – Parent/guardian service hours must be up to date — no holds allowed
2. No Outstanding School Fees – Scholar’s school account must be clear of all outstanding fees (e.g., uniforms, lost books, devices)

Participation Fee:

1. If your scholar is selected for the team, a $220 athletic fee must be paid before they are allowed to practice or participate in games or matches
2. Payment instructions will be provided with the acceptance notification
